Ok, I see a variety of different pills surrounding a tube of lipstick. The emptiness is how the person feels both on and off said pills, the lipstick being there to make the person perfect in their own mind.

I hope that made sense. Sometimes it's hard for me to explain what I seem to understand instinctively.
